Jaylan Phelps Cook

Jaylan Phelps Cook, 26, passed away on Friday, June 28, 2019 in Ridgeland, South Caroloina. He was born on January 3, 1993, in Evansville, Indiana, to Chanel Cook and Marcus Ivy.
He leaves to cherish his memories: two children, daughter Leilyn Cheeks and son Zaylan Cook; his parents; his longtime partner, Bria Smalls; his sisters, Gabrielle Griffin and Kaylayah Ivy; his grandparents, Esther Moore and Jerry Cook; his great-grandparents, Clara and Harold Jackson and Imogene and Jerry Lowery; his great-great grandmother Virginia Johnson; along with numerous aunts, uncles, cousins, and family friends.
Funeral Service will be held at 11:00 a.m. , Saturday, July 13, 2019 at Nazarene Missionary Baptist Church, 867 E Walnut St, Evansville, IN 47713, officiated by Reverend Larry A. Rascoe, Sr. Friends may visit Saturday from 11:00 a.m. to service time at the church. Interment will follow at Oak Hill Cemetery.
